The Florida High School Athletic Association (FHSAA) requires a completed Participation Physical Evaluation form and a Consent and Release from Liability Certificate to be on file with the Athletic Director prior to the student-athlete’s participation in any sport’s activity.
These athletic forms must be provided annually.

PLEASE NOTE: These two athletic forms are in addition to the two health forms required upon entry to the school, which are kept on file in the Registrar’s Office. (See High School Health Forms below.)
DEADLINE: Because of the large number of student athletes, FHSAA Health Forms must be received by Jesuit’s Athletic Director, Terry Rupp, no later than the Thursday before opening day practice in order for students to participate in that practice. If forms are turned in after that, they will be processed as they are received.

Schools are required by state law to have on file every student’s health record (Goldenrod Form DH3040, available only from a physician’s office or the health department) for the year of the student’s entry. We are also required by law to have a certificate of immunization (Blue Form DH 680, also available only from a physician’s office or the health department). Jesuit’s Registrar handles receipt of these forms. If these forms have not been received, the registrar will contact you. The Health Department will close down a school that does not have these forms on file for every student.
